Following your surgical consultation, you will be given a referral for admission form to complete. This form needs to be completed and returned to us prior to surgery. It allows us to confirm your admission details, medical history and insurance details prior to your admission. Can you please return the completed form to JSDS by one of these methods (please note if you fax or email the form we will require you to bring the original document with you on the day of surgery).
